Profile – Authorization forms

The choices made in this section apply to the following forms:

  • Representative Authorization Request (federal);
  • MR-69, Authorization to Communicate Information or Power of Attorney (Québec);
  • Alberta Consent Form (Alberta).
  • FIN 146, Authorization or Cancellation of a Representative (British Columbia)

Authorized representative identification

This subsection allows you to indicate in which forms the information it displays must be updated. By default, no check box is selected.

You can designate, for all client files, the person authorized to represent the corporation. The fields in this subsection are completed automatically with the information found in Section A. “Identification” of the PROFILE tab, when at least one check box is selected in the “Authorized representative identification” subsection. You can also use overrides if the desired information differs from the information entered. In addition, to provide the information in the format required in the Québec Form MR-69, complete the following fields: “Apt. or suite,” “Street number” and “Street name, P.O. box.”

Representative Authorization Request form

By default, the Authorization Form is not electronically transmitted. This section allows you to define, for all client files, the options relating to the transmission of this form and the authorization given on the latter. You can modify these choices for a particular client by accessing this client’s Authorization Form directly.

Subsections “MR-69 - Authorization to Communicate Information or Power of Attorney” and “AAUT – Alberta consent form”

These subsections allow you to define, for all client files, the options relating to the filing of these forms. You can modify these choices for a particular client by accessing this client’s Forms MR-69 and/or AAUT directly.
